The 34-year-old Capital Radio host was jogging along the banks on Monday evening when he saw a Labrador in the water struggling near Hammersmith Bridge.
Jordan North jumped in to save the dog managing to reach a nearby float lift with the dog.
Fortunately, a lifeboat crew was on exercise nearby and, after a member of the public flagged the "panicky" Jordan, were able to reach him three minutes later.
More members of the public helped by shining their phone flashlights on the radio star to help the lifeboat crew find him.
Jordan spoke about the ordeal on his Capital Breakfast show on Tuesday, showing his thanks to RNLI Chiswick for their help.
"I was getting a bit nervy as well because I thought my legs are going to go, I can't hold on much longer so they got there just in time."
"The real heroes here are the RNLI who came out and got me, put a blanket round me. They're the heroes."
The crew that saved Jordan was made up of Thames Commander Gavin Simmons, alongside Sid Blake, Tom Coe, and Cameron Crawley.
Gavin told The Sun: "We are on call to help all those who find themselves in difficulty on the water 24 hours a day, 365 days a year, even our beloved pets!
"While we'd always encourage people to call 999 and ask for the Coastguard instead of entering the water themselves, we're happy that in this instance we were able to help Jordan and the dog safely back to dry land."
