The final coroner’s report for Cory Monteith has been revealed by TMZ.
According to the report the Glee star was taking heroin and drinking champagne in his hotel on the night of his death.
Shortly after noon on July 13th, Monteith’s body was discovered alone in his hotel room in Vancouver, Canada.
According to TMZ, the report revealed that responders at the scene discovered a "spoon with drug residue and a used hypodermic needle."
Two empty champagne bottles and glasses were also in the room.
The coroner lists Cory’s official cause of death as "mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol," as well as stating that Monteith "had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs."
The report suggests that the star's tolerance for substance abuse may have been lower than usual because of a recent rehab stint in April.
Last month, Monteith was among the stars who were remembered in a special Emmy's “In Memorium” segment at the 65th Emmy Awards.

A tribute episode of Glee will air next week, on October 10th. Monteith was 31.
